Pre -Visit Requirements

  1. All patients must be 18 years or older. State issued photo ID will be required. Minor patients who are inquiring about treatment, please call the office directly.

  2. All patients seeking treatment by Serene Health must be a legal resident of MD and provide proof of residency.

  3. Must be diagnosed with a qualifying medical condition and have records documenting the diagnosis.

  4. Please confirm that a Serene Health Consultant has received your medical records prior to your appointment.
