Gathering Files

Electronic colored copy of a valid US government issued photo ID and proof of Maryland address.

Step 2

Register

Visit the MMCC Registry website and click the Create Account link under the Patients heading.

Step 3

Verify Email

After submitting your online application you will receive a verification email.

Click the link supplied in the verification email and verify your email address.

Once MMCC has reviewed you application they will notify you by email indication whether or not your application has been approved.

Step 4

Once approved call and schedule your appointment with Serene Health and see our Certified Physician to receive your written certification as long as all criteria has been met.
