Registering With Maryland Medical Cannabis Commission.

Registration for Adult Patients

  •  Adult Patients (18+ years of age):

Process to Register as an Adult Patient (18+ years of age)

  1. Gather the required ​files and information. As part of the online application for adult patients, you will need:

    • A valid, accessible email account which you are comfortable using for your Maryland Medical Cannabis account. The Commission will use this email address as the principal means of communication regarding your account and its status.

    • The last four digits of the patient's Social Security Number

    • An electronic copy of a valid US government-issued photo ID and proof of Maryland address or treatment at a Maryland medical facility. See acceptable Forms of identification below.

      • Driver's license or Motor Vehicle Administration identification card (both front and back)

      • Military ID

      • Passport

  • An electronic copy of a clear photograph taken within the last six (6) months. We strongly recommend using a free, highly-rated passport photo app widely available for download via the App Store, Google Play Store or anywhere apps are available.

Generally, the photo should look like a driver’s license or passport photo. Specifically, it must be a clear, color photo of the patient’s obscured face taken within the last six months to reflect the patient’s current appearance and feature a plain white or off-white background. The patient must be clothed, directly facing the camera with both eyes open and should have either a neutral facial expression or a natural smile.

  • Minor Patients (Under 18 years of age):

    Minor patients are required to have at least one caregiver assigned to their accounts at all times. Only parents or legal guardians 21 years of age and older are eligible to serve as caregivers for minor patients.

  • Caregivers must register with the Commission prior to registering the minor patient.

Registering a minor patient requires additional forms, one of which must be notarized, so please review carefully the requirements in the Process Overview and Quick Reference Card documents for registering a minor patient.
